Abstract

Secondary and tertiary medical care in Japan is provided and maintained by hospital doctors. Recently, the shortage of hospital doctors is becoming critical nationwide, which is severer in countryside. Historically, the progress and development in medicine has focused on mainly establishment of medical systems and introduction of most recent techniques and medical equipments; however hospital doctors’ work environment has been hardly improved following those factors. Accordingly, the work environment has become worse and worse to the point, where doctors could lose and/or abandon their mission, dedication or passion. Consequently, numerous doctors resigned hospital or tend to avoid choosing tough department, ex., surgery or internal medicine, inducing further doctors’ shortage in hospitals. In the present case study, a female dermatologist, working in a secondary hospital, re-consider 1) carriers as a hospital doctor of her own as well as many doctors around, 2) the problems of current medical systems, and 3) the reality of hospitals under the current systems. Then the author attempted to clarify a correlation and/or relationship between hospital doctors’ carrier or shortage and the current medical systems. Conclusively, the author indicated a proposal to solve the hospital doctors’ shortage so that Japanese secondary and tertiary medical care could be sustainably maintained under an appropriate work environment of hospital doctors.
